North Dakota H2 Action Planning Session Invitation





You are cordially invited to attend North Dakota's HAction Planning Session, part of the U.S. Department of Housing and Urban Development’s exciting H2 Initiative Technical Assistance series, designed to help communities integrate subsidized housing and health care services on a systems level to better serve (1) people experiencing homelessness and (2) low-income people living with HIV/AIDS.


North Dakota is fortunate to have been selected by HUD as one of only twenty sites to receive an on-site TA session!

This is a fantastic and unique opportunity for you to contribute to the design and development of North Dakota's new Action Plan to improve access and effective utilization of health care services for people experiencing homelessness, as well as low-income people living with HIV/AIDS. Participants will engage in expert-facilitated collaborative action planning sessions and develop specific strategies to implement throughout 2016.

Key Features of H2 Action Planning Sessions
  • Participation by key stakeholders from both the Homeless Assistance and Health Care systems, including participants from all levels: agency and program staff, from front-line workers to Executive Directors; city, county, and state-level government representation, Federal Agency representatives (HUD, SAMHSA, HRSA, USICH, CMS)
  • Knowledge exchange across the homeless assistance and health care systems, including baseline knowledge of how each system operates and opportunities for connection and integration, including overviews of promising programs and initiatives from within North Dakota, funding opportunities, and best practices/models from around the country
  • Subject matter experts and experienced, effective facilitators working with diverse group of participants, focused on eliciting actionable strategies to address the specific needs of your communities
  • Facilitated breakout discussions to elicit strategies and action steps for North Dakota's H2 action plan, which the TA Team compiles into a draft action plan overnight. On the second day, participants are directly involved in reviewing and revising the emerging draft action plan
  • Consideration of and alignment with existing complementary structures, strategic plans, and initiatives
Outcomes
  • Connections to people serving the same populations you serve
  • Increased knowledge of resources in your community, on both the housing and health care side, and how to help your program/organization/clients access those resources
  • Increased knowledge of potential funding sources to tap, both within and outside of your state
  • A draft action plan to improve health and housing outcomes and stability for people experiencing homeless, as well as low-income people living with HIV/AIDS, by integrating housing assistance and health care systems

  • What health services and treatment are afforded these two populations and how?
  • What is the gap/unmet need?

4.  Integrated Data Collection and Analysis: Identifying the Population at the Intersection of Housing and Health Care Assistance

  • Integrating data collection: Identification of data needed to inform planning and improve health care access.
    • Who are the members of these populations? How do we identify them?
    • What do we know about their housing and health care needs?
    • What are our data sources and systems?
    • What challenges do we face in collecting/sharing data across health care and housing systems?

5.  Core Issues in Program Design and/or Medicaid Change: Developing a Concrete Action Plan to Close Identified Gaps
  • Review of Draft Action Plan emerging from first day
  • Creation of Goal/Objective statements
  • Revision of strategies/action steps presented, including clarification, consolidation, and additional details
  • Identification of priorities
  • Consideration of cross-cutting issues necessary for implementation:
    • Training: Will staff need to be trained?
    • Partnerships: Are partnerships needed to accomplish plan objectives?
    • Targeting: Should particular subpopulations be targeted?
